Case Manager – Emergency Assistance – Tulsa

Reports to: Director of Emergency Assistance

Position Description

This full-time, benefited position will provide case management services to clients as a part of a case management team and will be cross-trained and able to ‘deploy’ to assist in various departments around campus as the need arises. This individual will perform needs assessments, develop and implement individualized service plans with goals, advocate for and refer clients to relevant community agencies, maintain client files, carry an appropriate caseload of 25-35, make home visits and assist with data collection and reporting all with the goal of household stability, self-sufficiency and ultimately household flourishing.  

The purpose of the Emergency Assistance program is to support and assist households to meet their basic needs, gain stability and achieve their full potential. 

Ministerial 

  • As a ministerial representative of CCEOK, it is expected that both professional and social behavior be aligned with the ethical and moral standards of the Catholic Church 
  • Pray daily for the needs of the poor; opportunities include personal prayer, daily Mass, and/or adoration 
  • Willingness to learn how to articulate Catholic teaching with a commitment to its application in the job function 
  • Execute duties in a Christ-like manner with love and compassion following the values and principles of the program, Catholic Charities and Catholic Social Teaching 

 

Key Duties 

  • Develop trust with clients to provide advocacy and direction to ensure the clients have the best means to reach self-sufficiency 
  • Perform comprehensive strength needs assessments 
  • Determine the nature of the clients’ situations through client interviews and visits 
  • Create individualized service plans and goals 
  • Assist clients in achieving goals by helping them create achievable steps 
  • Refer clients to internal and external community resources  
  • Enroll/register in social programs such as HUD housing, SNAP, WIC, SoonerCare
  • Provide extensive case management, including minimal bi-weekly contact with each client combined with phone and face-to-face office and home visits
  • Record client progress including referrals, goal achievement, visits and all interactions 
  • Obtain proper documentation for files
  • Maintain a proper active caseload and review weekly with the director  
  • Evaluate client progress regularly adjusting service plan as needed to improve outcomes 
  • Encourage their spiritual growth and invite them to learn about the transforming power of God 
  • Engage in ongoing training and stay up to date on current community resources for clients 
  • Other duties as assigned 

 

Key Skills 

  • Bilingual in English and Spanish preferred 
  • Compassionate and ability to work with people of economically and culturally diverse backgrounds 
  • Bachelor’s degree in social work preferred or equivalent 3 years work experience in case management services 
  • Strong time management skills and ability to prioritize workload 
  • Excellent written skills and verbal communication skills including telephone and email communications 
  • Motivational interviewing skills 
  • Critical thinking, problem-solving skills and decision-making ability to determine the best service plan for each client 
  • Ability to prioritize and manage multiple tasks and responsibilities and meet deadlines 
  • Proficient in computer usage including a demonstrated proficiency in the use of Outlook, PowerPoint, Word, Excel, and Office365
