Position Description
This full-time, hourly, benefited position provides compassionate, strengths-based case management to individuals and families experiencing crisis and economic hardship. The Case Manager conducts comprehensive assessments, identifies opportunities for client growth, removes barriers to success, and develops individualized service plans that promote long-term stability and self-sufficiency. Through resource navigation, advocacy, and ongoing support, the Case Manager empowers clients to achieve their goals while ensuring professional, timely, and effective service delivery.

Ministerial
- As a ministerial representative of CCEOK, maintain professional and personal conduct that aligns with the ethical and moral standards of the Catholic Church
- Pray daily for the needs of the poor through personal prayer, daily Mass, and/or adoration
- Be willing to learn and apply Catholic teaching in this role
- Help foster a shared culture of ministry across program operations and management
CLIENT INTERACTION / CASE MANAGEMENT
Client Engagement & Assessment
- Conduct comprehensive strengths-based assessments to identify immediate needs, barriers to stability, and opportunities for long-term growth
- Engage clients in a collaborative service-planning process that promotes ownership of goals, action steps, and target completion dates
- Work one-on-one and/or in group settings with Transitional Living (TL) parents, particularly fathers, to promote family and household stability
- Collaborate closely with the TL team to ensure that the whole family receives a holistic, coordinated apprach that addresses their individual needs, strengths, and goals
- Utilize motivational interviewing, problem-solving frameworks, and clinically informed interventions to support client engagement and progress
- Maintain a non-judgmental, compassionate approach while serving individuals and families from diverse economic, cultural, and social backgrounds
- Conduct ongoing assessment of client progress and adjust service plans as needed to promote success
Case Management & Advocacy
- Develop, implement, and monitor individualized service plans focused on financial stability, self-sufficiency, and overall well-being
- Empower clients to identify and pursue multiple pathways toward achieving their goals
- Advocate for clients and connect them with internal and community resources, including housing assistance, employment services, childcare, healthcare, mental health services, education, and other supportive programs
- Educate clients on budgeting, financial management, and available community resources
- Maintain professional boundaries while fostering trusting and supportive relationships with clients
Documentation & Compliance
- Verify all required documentation, including proof of income, residency, household composition, and utility statements, ensuring compliance and accuracy
- Complete timely, accurate, and thorough documentation in all required databases and case management systems, including Blackbaud and Apricot
- Support program evaluation and reporting requirements through accurate recordkeeping and data collection
- Maintain confidentiality and comply with agency policies, ethical standards, and applicable regulations
COMMUNITY OUTREACH & COLLABORATION
- Build and maintain collaborative relationships with community agencies, service providers, and faith-based partners
- Collaborate with internal teams to support organizational goals and ensure seamless service delivery
- Maintain current knowledge of social work practices, community resources, poverty-related issues, and emerging trends affecting vulnerable populations
QUALIFICATIONS / EDUCATION
- Master’s degree in social work, Clinical Social Work, Counseling, Human Services, or a related field preferred; bachelor’s degree in social work or related field required, or equivalent combination of education and experience
- Minimum three (3) years of experience in case management, social services, crisis intervention, or a related human services field preferred
- Bilingual in English and Spanish preferred
- Ability to articulate Catholic teaching and integrate Catholic Social Teaching into service delivery
- Demonstrated knowledge of strengths-based case management, motivational interviewing, crisis intervention, and client-centered service planning
- Strong critical thinking, problem-solving, and decision-making skills
- Excellent written, verbal, telephone, and interpersonal communication skills
- Ability to manage multiple priorities, maintain accurate documentation, and meet deadlines
- Proficiency in Microsoft Office Suite, Outlook, Excel, PowerPoint, Word, Office 365, and electronic case management systems
- Reliable transportation, valid driver’s license, and ability to travel within the service area as required
