Eligibility Requirements
To be considered for assistance, applicants must meet the following criteria:
Case management services are available for individuals and families who are interested in additional support. Many of the individuals and families who connect with our program may benefit from working with a case manager, and we encourage you to explore how case management can support your goals.
Our strength-based case management approach focuses on partnership. You’ll work together with a case manager to identify your strengths, set personal goals, navigate challenges, and create a plan that is tailored to your unique needs. Our goal is to help you build stability, address financial stressors, and work toward preventing homelessness.
